Client Background Information Form

Two types of forms are available from this page:

Link to background form in Word format. A Word form, which can be filled-in and emailed to Jane.  If you have Microsoft Word on the computer that you are using, this is the best form for you.  Click on any of the Word icons on this page to download this form.

Link to background form in Acrobat format. An Adobe Acrobat form, which needs to be printed, filled-in, and then mailed, faxed, or brought with you for your first session. Click on any of the Acrobat icons on this page to download this form.

How to use the Word Form:

  1. Click the Link to background form in Word. Word link, and the form will open on your Desktop.
  2. Fill-in the fields with your information.
  3. Save the completed form to your Desktop by going to File > Save As...
  4. Choose your Desktop as the place to save the form.
  5. Choose a name for your file.  Example:  JaneEllen.doc
  6. Email the form to Jane as an attachment.

How to use the Adobe Form:

  1. Click the Link to background form in Acrobat format. PDF link, and the form will open on your Desktop.
  2. Print the form.
  3. Fill-in the fields with your information.
  4. Fax, mail, or bring the completed form with you for your first session.
